Bug Reporting Format Requirements

When reporting a bug, please provide the following information. You may copy and paste below into a new post and edit the fields as necessary.
Subject: Bug - [Short description - such as Can't Save Availability Update]
Link to page where bug noticed: Copy the link from your browser address bar
Device Used: PC, Mac, iPhone, Android Phone, etc
Web Browser and Version if known: Chrome, Firefox, Edge, Safari etc.
Critical Failure?: Indicate if this caused an error such as 500 Internal server error or caused the program to stop operating
What Happened?: Describe what happened and how it differed from the expected result.
Steps to Reproduce: Provide the steps taken to reproduce the error
Screen Shot: Attach a screen shot if you believe it will help troubleshooting
